We present a 32-year-old woman with a parasagittal white matter sheari
ng injury of the left frontal lobe following a motor vehicle collision
. There was monoplegia of the right lower extremity. Computed tomograp
hy failed to demonstrate the lesion which was visible on magnetic reso
nance images. In particular, fluid-attenuated inversion recovery image
s were useful to depict the shearing injury, since sulci were of low i
ntensity on those images.
