Background- Transanal microsurgery, endoscopic laser photocoagulation and s
nare resection have all been used to treat large sessile :adenomas of the r
ectum alternatively to surgical approach. However, these modalities are oft
en defective due to the carpet-like shape and the frequently large extensio
n of the lesions,
Methods: twenty-two patients with carpet-like adenoma (C-LA) have been subm
itted to transanal endoscopic resection (TER) by uro:logical resectoscope.
Results: Complete eradication was obtained in 16 patients (follow-up 12-24
mts) and 6 patients are still treated (follow-up 3-6). The mean number of t
reatment sessions was 3 (range 1-6), The mean time tween the first treatmen
t and the complete eradication was 6 months (range 1-18). In three patients
intraoperative bleeding have been stopped by local injection of epinephrin
e 1:20.000 and patients have had four early postoperative bleeding (within
72 hrs). No early or late mortality related to the procedure was reported.
Conclusions: This preliminary experience shows the efficacy of transanal en
doscopic resection by means of urological resectoscope in the treatment of
carpet-like adenomas of the rectum.
