Programmed transesophageal cardiac pacing revealed electrophysiological sig
ns suggesting longitudinal dissociation of the atrioventricular (AV) node i
nto multiple pathways in 7,7% of 147 patients with WPW syndrome and induced
paroxysmal orthodromic AV reciprocating tachycardia. Electrophysiological
mechanisms of participation of rapid and slow conduction pathways within AV
node in initiation of orthodromic tachycardia were established. Variants o
f participation of rapid and slow conduction pathways within AV node and of
extranodal pathways in formation of orthodromic AV reciprocating tachycard
ia were distinguished.
