We report a case of acute pneumonitis with pleural effusion and respiratory
distress sydrome that was the inaugural sign of leptospirosis in a 37-year
-old patient exposed to mt dejections at home. The patient was given penici
llin and oxygen therapy with evacuation of the pleural effusion. Lung manif
estations in leptospirosis usually occur as non-specific cough and hemoptys
is. Pleural effusion is uncommon. Adult respiratory distress syndrome and p
rofuse hemoptysis can also occur requiring special care.
