Duodenal malignancy is rare and generally considered to have both a lo
w resectability rate and a poor prognosis, Historically, the involveme
nt of the colon or its mesentery has been considered a criterion for u
nresectability by many surgeons because of the overall magnitude of su
rgery involved with an en bloc colectomy and pancreaticoduodenectomy.
In the past few years, several reports have noted a decrease in morbid
ity and mortality rates for pancreaticoduodenectomy. The current safet
y of the procedure suggests that the classical criteria for resectabil
ity can now be reevaluated for certain neoplasms, We report two cases
of pancreaticoduodenectomy with en bloc colectomy done as attempted cu
rative resections for primary duodenal malignancies, The procedure was
well tolerated by both patients; there were no major complications, a
nd it provided both prolonged survival and effective palliation.
