Abstract
Information and counselling are essential prerequisites for self-determinat ion in old age. For the offers available the following differentiations are made: general oral, written or electronic information on supporting servic es, individual personal information and counselling, individual accompanyin g counselling and individual written information on the own health or disea se situation. Models of good practice in Hamburg are presented as examples.
