Laparoscopic management of renal cystic disease is a safe and effective met
hod of treating symptomatic cysts, which are refractory to other forms of t
reatment. In fact, it is not only a safe alternative to open surgery, but t
he ideal choice for simple renal cysts. For symptomatic peripelvic cysts, i
t is an effective technique for management but needs skillful and challengi
ng dissection. Laparoscopic cyst marsupialization in patients with autosoma
l dominant polycystic kidney diseases (ADPKD) can effectively reduce the pa
in, drain the abscess, relieve the hypertension, and stabilize the renal fu
nction. The approach-transperitoneal or retroperitoneal-depends on anatomic
location and the surgeon's preference and expertise. The long-term results
obtained in the author's experience offer a strong argument favoring retro
peritoneoscopic surgery in patients with symptomatic renal cysts.
