We describe a 51-year-old woman with an acquired coronary fistula to the le
ft ventricle. Reports in the Literature about acquired coronary fistulas to
the left ventricle are scarce. In this patient, the fistula developed afte
r septal myectomy for hypertrophic obstructive cardiomyopathy, Transesophag
eal echocardiography may be the preferred method to diagnose and evaluate t
hese fistulas. Moreover, in contrast to fistulas to the right ventricle, co
nservative management carried a good prognosis in this patient.
