We report the first case of foreign-body granuloma occurring in the mandibl
e of a patient with a history of orthognathic surgery. A 20-year-old male p
atient had a hard swelling and pain in the left mandibular angle and the pa
rotid area. Plain radiographs showed a radiolucent lesion extending from th
e buccal area under the left sigmoid notch oi the mandible to the mandibula
r angle. Computed tomographs showed an expanded osteolytic mass in the same
area, with destruction of most of the bone on the buccal side. A microscop
ic examination revealed filamentous foreign material associated with chroni
c inflammatory cell infiltration and an aggregation of multinucleated giant
cells. Foreign-body granuloma in the mandible, although rare, should be in
cluded in the differential diagnosis in cases of postoperative masses.
