Abstract
The authors linked admission risk factors to a series of indicators for com plex care delivery to enable detection of patients in need of care coordina tion at the moment of admission to the ge; end hospital. The authors found 13 risk factors to be predictive of more than one indicator of care complex ity. An admission risk screening procedure to detect patients in need of ca re coordination should focus on these risk factors and should include predi ctions made by doctors and nurses at admission and information collected fr om the patient and the medical chart.
