Endoscopic trans-septal frontal sinusotomy (TSFS) represents a unique surgi
cal approach to the floor of the frontal sinus. Although the final result c
an have similarities to the modified Lothrop procedure in that the intersin
us septum may be drilled out, endoscopic TSFS represents a novel approach t
hat can he valuable in patients with certain anatomic configurations. Endos
copic TSFS represents an alternate approach to the frontal sinus that may b
e used by the experienced endoscopist to augment treatment of refractory fr
ontal sinus disease. This procedure is best considered for patients with fa
vorable anatomy who have significant frontal sinus disease and cannot be ma
naged adequately through an endoscopic frontal sinusotomy.
