The use of gadolinium-enhanced magnetic resonance (MR) to isolate an infect
ed renal cyst ill the Setting of autosomal dominant polycystic kidney disea
se (ADPKD) has not been previously described. A case in which T1- and T2-we
ighted and gadolinium-enhanced MR images were used to identify a single pur
ulent cyst in a patient with ADPKD is herein presented. We Suggest that gad
olinium-enhanced MR be considered useful in the evaluation of ADPKD patient
