Coccidioidomycosis is an endemic fungal infection of the southwestern Unite
d States. Normally a self-limited infection in healthy hosts, coccidioidomy
cosis can become a serious complication in patients who have had solid orga
n transplantation. Among patients whose solid organ transplantation was com
plicated by coccidioidomycosis, the infection has a variety of clinical pre
sentations. Disseminated disease is common and has substantial morbidity. P
atients at risk for coccidioidal infection should be identified so that ant
ifungal prophylactic therapy can be initiated. Treatment options include am
photericin B or azoles. Secondary prophylaxis is recommended because relaps
e is frequent.
