We describe a case of tuberculous peritonitis with unusual manifestations i
n a continuous ambulatory peritoneal dialysis (CAPD) patient. In such patie
nts, tuberculous peritonitis typically produces a neutrophil predominance i
n dialysate differential counts. In the presented patient, dialysate analys
is revealed only lymphocyte predominance. Additionally, his condition impro
ved, with no white blood cells found in dialysate samples after empirical c
efazolin and gentamycin antimicrobial therapy. Nevertheless, dialysate samp
les did yield Mycobacterium tuberculosis during the sixth week of hospitali
zation, and a diagnosis of of tuberculous peritonitis was made. This case s
hows that tuberculous peritonitis should not be excluded in a CAPD patient,
even when showing a satisfactory clinical response to empirical antibiotic
