A 47-year-old human immunodeficiency virus-seronegative West African man wh
o presented in extremis with cachexia, lymphadenopathy, multiple organ dysf
unction, and marked T-lymphocytopenia received the diagnosis of disseminate
d tuberculosis, cryptococcal pneumonia, and cryptococcemia. His subsequent
course and our review of the literature suggest that the profound T-lymphoc
ytopenia and ensuing cryptococcal disease were likely attributable to disse
minated tuberculosis.
