A 71-year-old man with acrodermatitis continua of Hallopeau was treated suc
cessfully with a combination of oral propylthiouracil and methotrexate. Aft
er 14 weeks, he developed acute pancytopenia, an uncommon idiosyncratic sid
e-effect of propylthiouracil, and presented with a life-threatening methici
llin-resistant Staphylococcus aureus pneumonia. This illustrates the potent
ial value and associated risks of propylthiouracil in the management of thi
s difficult condition.
