In four patients with acute basilar artery thrombosis, complete arterial re
canalization and good neurologic outcome were achieved with a treatment com
bining alteplase with tirofiban. In no cases were cerebral or extracerebral
hemorrhagic complications observed. Combined fibrinolytic agents and glyco
protein IIb/IIIa inhibitors may have high potential in the treatment of acu
te cerebrovascular thrombosis.
