We describe a patient with painful enlargement and severe nail deformi
ty of both great toes. The patient furthermore presented with some min
or diagnostic criteria for psoriasis such as nail pitting and bad a po
sitive family history of the disease. In the recent literature, such c
ases have been recognized as a new entity termed psoriatic onycho-pach
ydermo-periostitis (POPP). In this case report, POPP is discussed in r
elationship to other forms of psoriatic arthropathy and with special e
mphasis regarding its diagnostic criteria and therapeutic implications
