PURPOSE: To report treatment of a patient with acquired immunodeficien
cy syndrome (AIDS) and ocular and paranasal sinus microsporidial infec
tion. METHOD: Case report. RESULTS: A patient with AIDS and ocular mic
rosporidial infection experienced resolution of ocular symptoms with t
opical fumagillin, but symptoms recurred upon cessation of therapy. Pa
ranasal sinus microsporidial infection was diagnosed. The patient rece
ived sequential systemic treatment with itraconazole followed by alben
dazole. Sinus symptoms resolved with albendazole, He remained symptom-
free with a normal examination 17 months after concluding therapy. CON
CLUSIONS: Although fumagillin and itraconazole may have played a role,
systemic albendazole appears to be responsible for clinical resolutio
n of microsporidial infection.
