This case report describes an atypical form of acute pulmonary allograft re
jection that was refractory to conventional therapy. The rejection manifest
ed primarily as interstitial lymphocytic infiltrates with little perivascul
ar involvement. Despite aggressive therapy the patient died within 7 months
of transplant. The timely recognition and treatment of unusual forms of al
lograft rejection is vital in the management of pulmonary transplant patien
ts.
