A case of 64-year-old patient is reported in whom multiple myeloma with Ben
s-Jones proteinurea stage IIIB ran in the period of making diagnosis by typ
e of plasm cell leukemia. In diagnosis, besides common clinical and cytomor
phological methods use was made of cytochemical, electron-microscopic and e
nzyme immunoassays of plasm blood cells and cells of bone marrow which unde
r light microscopy were characterized with some features of atypism peculia
r for immature (blast) cells. Immunophenotyping referred them to cells of t
he pathological population at the end stage of B-cell differentiation. Resu
lts of electron microscopy were also typical for cells well differentiated
in the direction of protein synthesis (Ig). Clinically, there were severe d
ysfunctions of the kidneys and liver which resulted in a lethal outcome. Th
e patient received therapy against chronic renal failure including 41 proce
dures of plasmapheresis. Plasm cell leukemia was treated with alkeran, vinc
ristine, prednisolone, program M-2 without noticeable effect. The patient d
ied in coma and gastrointestinal hemorrhage. The survival from the time of
the diagnosis was 4 months. Autopsy confirmed the diagnosis.
