Midgut rnalrotation is defined as a developmental anomaly that may cause at
ypical clinical symptoms in relatively common intestinal disorders due to a
ltered anatomy. A 27-year-old woman presented with acute left-sided abdomin
al pain. Underlying type la malrotation prevented the correct clinical diag
nosis of perforated, ulcerated appendicitis. Cross-sectional imaging demons
trated all the typical signs of this type of malrotation, i.e., right-sided
duodenojejunal junction, left positioned cecum and ascending colon, invert
ed position of the superior mesenteric vessels, and hypoplasia of the uncin
ate process of pancreas, and surgical treatment was initiated.
