Over the years, much has been learned from unraveling the pathophysiol
ogical alterations associated with the hypertensive diseases. Despite
this large base of fundamental and clinical information, our knowledge
continues to expand. This article discusses the multifactorial nature
of hypertensive disease, including the vascular and cardiac participa
tion in the elevation of arterial pressure and in target organ involve
ment by the disease. Some of the most exciting advances of the last de
cade are summarized in this review.
